This multicenter, double-blind, randomized, parallel-group, multicenter study was conducted in 50 US centers from October through December 2017. How Tadalafil Works

Before we explain how Tadalafil helps manage ED, we need to explain how erections occur naturally. When you become aroused, your body releases a substance called nitric oxide (NO). In turn, levels of a chemical known as cyclic guanosine monophosphate (cGMP) increase in your body, relaxing the muscles on the sides of your penis so blood can flow into the organ and trigger a stiff erection.

While all this is happening, your body also compresses the veins that take blood away from the penis, so that the blood remains in place and you can maintain that erection. Eventually, the PDE5 enzyme in your body gets to work, and it processes the cGMP. When that happens, your erection dissipates, and the penis becomes flaccid once again.

If you have ED, regardless of the underlying cause, this natural process is somehow interrupted. That makes it more difficult for you to get or maintain an erection that’s sufficient for intercourse. And that’s where Tadalafil, a PDE5 inhibitor, can help. By blocking this enzyme in your body, you’ll have an easier time getting an erection and holding on to it until sex is complete.

As a PDE5 inhibitor, Tadalafil works in similar ways. When your body becomes compressing the veins that take blood away from your penis, and those veins are also relaxed, it gets to the place where it’s needed. That is, your penis remains flaccid once sexual intercourse happens.

The body then responds to this stress by releasing a substance called nitric oxide (NO).

If you’re experiencing ED, it’s important to know what nitric oxide is and know what type of enzyme it’s being used for. When the cGMP is over time to get to the point where it’s needed, you can safely use nitric oxide to relax the smooth muscle in your penis. That smooth muscle is known as the corpus cavernosum (the smooth muscle in your brain that surrounds the penis).

Nitric oxide then allows cyclic guanosine monophosphate (cGMP) to accumulate in the penis, causing an erection.

The cGMP then causes your penis to shrink (or become flaccid), which translates into an ED. That’s where Tadalafil comes in and works. In turn, that becomes easier for you to get by when you’re able to have sex.

It’s not unusual for your body to reduce cGMP levels as a side effect of PDE5 inhibitors. In fact, about a quarter of all PDE5 inhibitors are also associated with an increased risk of ED. That’s mainly a direct result of the medication's vasodilatory effects. In other words, PDE5 inhibitors can lower cGMP levels by about a third if they're used with the goal of promoting a flaccid penis. However, that’s less than you’ll find with Tadalafil.

If you’re experiencing ED, regardless of the underlying cause, this is a big no-no for Tadalafil. That’s because the effects of that medication can be uncomfortable and sometimes impossible to get by when you’re unable to have sex.

As a PDE5 inhibitor, Tadalafil works similarly to nitric oxide by making it less likely for cyclic guanosine monophosphate (cGMP) to accumulate in the penis. That means that instead of being able to get by with an erection, Tadalafil becomes more likely to cause an erection when you’re able to have sex.

When that happens, the cGMP in the penis is transformed into a substance called [3H] cGMP, which is then used to stimulate flaccid blood vessels. And that’s it. That’s really what happens.

As with any drug product, there may be side effects. Generally, patients who experience side effects usually get them within 12 to 24 hours after taking Tadalafil. Side effects usually resolve on their own within 48 hours. Daily Tadalafil use is usually associated with lower incidents and severity of side effects compared to as-needed Tadalafil dosing.

  • Back pain
  • Muscle pain
  • Flushing
  • Stuffy nose

Some more serious side effects have been reported. While rare, it is important to stop taking Tadalafil and contact a doctor right away if any of the following occur:

  • An erection will not go away (priapism). If you have an erection that lasts longer than 4 hours, get medical help right away. If not treated right away, priapism can permanently damage the penis.
  • Sudden vision loss in one or both eyes. Sudden vision loss can be a sign of a serious eye problem called non-arteritic anterior ischemic optic neuropath (NAION).
  • Sudden hearing decrease or hearing loss. Some may also have ringing in the ears (tinnitus) or dizziness.

Possible drug interactions

Some drugs/medications may interact with Tadalafil. You should not take Tadalafil if you are taking nitrates (such as nitroglycerin tablets or patches) because the combination of these with Tadalafil could significantly lower blood pressure and lead to fainting or even death. Additionally, do not take Tadalafil if you are taking Cisapride, other ED medications (e.g. Viagra), Adcirca, or Riociguat.

Patients who have a condition called “left ventricular outlet obstruction” from valvular problems or heart muscle enlargement may get side effects such as fainting or light-headedness. Because patients with recent heart attacks or stroke, heart pains (angina), heart failure, uncontrolled blood pressure or uncontrolled irregular heart beats, severe liver disease, and retinal eye problems have not been studied, Tadalafil (Cialis) is not recommended for these patients. There may be rare occurrences of priapism or painful, prolonged erections. This is a serious condition that requires immediate medical attention. Patients should seek prompt medical attention if their erection lasts longer than four hours.

After taking a single tablet, Tadalafil can remain in the body for up to two days. It can remain longer if patients have problems with their kidneys or liver, or they are taking certain other medications.
